Admission Procedure

Procedure

  • Candidate seeking admission should apply on the application form supplied by the institute office with the Prospectus.
  • Candidate must have passed in all the required subjects separately. (in Theory and Practical)
  • In case the information furnished or certificates, etc. submitted are found to be false, or the candidate is found to have withheld or concealed any information in his / her supplication form, he / she shall be removed from the institute rolls and fees deported by his / her shall be forfeited.
  • Once application form is submitted, no alterations will be allowed thereafter.
  • All admissions are provisional subject to the verification of original documents and medical examination conducted by institute authorities.
  • No admission test ticket will be sent to the candidate if found not eligible or if application is incomplete. If the candidate does not receive the admission test ticket by the prescribed date he / she may contact the Institute Office immediately between 10am to 1pm. If found eligible, a duplicate admission ticket will be issued. The applicant must bring an attested copy of photograph similar to the one pasted on the application form or issue of duplicate admission test ticket.

Directions for filling-up Forms

  1. The application should be sent only the prescribed forms supplied along with the Prospectus. This can be had on sending a Demand Draft for Rs. 500/- payable in favor of "Lord Buddha Healthcare Foundation". The application form should be duly filled in by the applicant in his / her own handwriting and forwarded to the Principal, Sumitra Institute of Nursing and Paramedical Sciences, Greater Noida, Gautam Budh Nagar (U.P.).
  2. Attach the required documents in the following order:-
    1. Acknowledgment Card affixing five rupees stamp.
    2. Self addressed envelope (24cm x 18cm) with postal stamp.
    3. Attests copies of following certificates: Original Certificates are to be submitted at the time of admission.
    4. Secondary school Certificate / Mark Sheet & admit card mentioning date of birth.
    5. (10+2) Certificate and Mark Sheet.
    6. Medical Certificate from a Registered Medical Practitioner holding a degree not below M.B.B.S.
    7. Three recent passport size photograph attested by Principal of School last attended /Gazetted Officer. One photograph to be pasted on the Application form and six extra photographs for office work.
    8. Migration Certificates.
    9. Domicile of Uttar Pradesh for ANM candidates.

Rules and regulations

  1. All students admitted in the institute must observe the rules and regulations that may be framed from time to time by the institute authorities. Students are required to maintain absolute self discipline in the institute campus. Violation of rules will result in severe disciplinary action against the offender including his / her suspension or expulsion from the institute. In such cases he / she will forfeit all the deposits and shall not be eligible for any claim.
  2. Each student is responsible for the proper handling and safe custody of any apparatus or equipment that he / she may be using. In case of any damage or loss to the institute property through misuse or negligence, the student shall have to pay the cost of repair or replacement.
  3. Students must show respect and obedience to all members of the staff as well as managing authorities of the institute.
  4. Every student shall have to attend classes and practical.
  5. In case of illness, permission will have to be obtain from the Principal for remaining absent from studies.
  6. All dues have to pay according to Fee Payment Schedule.
  7. Students are advised not to process on leave without permission of the Principal, otherwise they will have to pay Rs.100/- day during the period of their unauthorized leave. The will have to report daily in the Institute attendance.
  8. The Principal of the institute reserves the right to rusticate any student found indulging in acts prejudicial to the interest of the institute or involved in gross indiscipline or immortal acts.
  9. Hostel residents are instructed to keep their rooms neat, clean and tidy. Warden / Officer the hostel may check rooms at anytime; if found guilty, the erring student may be warned or punished.
  10. Students are not to keep any medicines in their rooms except during illness.
  11. Parents and natural guardians of the hostel residents can appoint local guardians, if they so desire, to meet the students on permissible days and time. In case of appointment of Local Guardians, their entire personal data along with photograph shall have to be submitted. On the request of local guardian, if the student is allowed to go out of hostel, the responsibility would be on the parents who appointed the local guardian.
  12. All the students of the hostel are supposed to be in the hostel before 09:00pm. There will be attendances at 09:00pm every day. In special circumstances, with permission of the Principal, the student may be allowed to stay outside for a day or two.
  13. No student can claim room of choice, electric appliance, almirah, stool and chair, etc. other than provided by the institution.
  14. Morning and evening prayers will be held in the Assembly Hall in the Campus. Formal prayer is compulsory.
